When held by a Pokémon, it raises its Coolness in the primary round of a Pokémon Contest, the Visual Competition of a Super Contest, and the Introduction Round of a Contest Spectacular.

In Generation III, it is raised by 20 points, up to a total of 255 points.

Before competing in the Master Rank Contest, Ruby was given all five scarves, which he used to enhance the respective conditions of five of his Pokémon, leading all of them to win the Contest and fulfilling Ruby's part of the 80-day challenge with Sapphire. He used the Red Scarf to enhance Nana's Coolness.

Trivia

The various Manaphy distributions having a Red Scarf as a held item may be a reference to May's bandana held by Manaphy in the movie Pokémon Ranger and the Temple of the Sea.
