A phaser report was a report on the status of a starship's phasers.

In 2267 of the mirror universe, Montgomery Scott, masquerading as his mirror self, reported to the real Captain James T. Kirk, also in the place of his double, that ISS Enterprise's phasers had suffered no damage. (TOS: "Mirror, Mirror")
