Personnel application

A personnel application was a form filled out when applying for a job.

In 2367, during a hearing Sabin Genestra accused Simon Tarses of lying when filling out his job application, by claiming Vulcan heritage rather than Romulan. He admitted as much to Jean-Luc Picard, saying it was a mistake. Picard later told Norah Satie. (TNG: "The Drumhead")
