Design flaw
A design flaw was a weakness or imperfection in a product that was built into the product during the design phase.
In 2365, the crews of both the USS Yamato and the USS Enterprise became concerned that there might be a design flaw innate to Galaxy-class starships after the Yamato suffered a series of system malfunctions which then spread to the Enterprise adn which ultimately destroyed the Yamato. Ultimately, however, the source turned out to be an Iconian software transmission that was incompatible with the vessels' systems. (TNG: "Contagion")
