The lives of these three officers were claimed in a fatal accident due to pilot error on the part of Tom Paris, the cause of which was unable to be determined independently. Paris initially denied responsibility for the accident, but later admitted he had falsified reports to hide his culpability. (VOY: "Vis à Vis") Haunted by their deaths, he eventually admitted his mistake and was expelled from Starfleet.

When discussing his actions with Harry Kim on USS Voyager in 2371, Paris referred to his confession as the "worst mistake I ever made". (VOY: "Caretaker")

Later, in 2373, after Neelix asked Paris the circumstances of what lead to his time in prison, Paris admitted that "I've thought a lot about that, and it comes down to one simple fact. I didn't tell the truth. I made a mistake, which happens to people, but if I'd admitted that mistake it would have been a lot better. But I lied about it, and it nearly ruined my life." (VOY: "Fair Trade")

These officers were only mentioned in dialogue.
The Pocket VOY novel Pathways gives them the names Brunolf Katajavuori (β), Odile Launay (β), and Charlie Day (β). It also reveals that Paris initially blamed the crash on Katajavuori rather than himself, before confessing to his role eight months later.
