- Complete Monster - Sosa is revealed to be this, both in the movie and in the game! "It is necessary that you do kill children. That way, heroes don't get confused and go on 60 Minutes. They instead crawl back into the holes they came from."
- Crowning Moment of Awesome - The opening level is the mansion gunfight. You have your little friend with infinite ammunition. It's time to clean house.
- And one highlight is having Tony turn around and change history by wasting The Skull.
- After the Diaz brothers try to have Tony whacked and kill his mom, Tony drives to their place and pulls a chainsaw out of the trunk of his car. He then walks right into their operation, revs up his chainsaw, and screams "You and your fuckin' brother are DEAD!" Cue Roaring Rampage of Revenge.
- Demonic Spiders – Bazooka, chainsaw and Grenade Launcher users. Also the Pirates in the boat smuggling runs.
- Esoteric Happy Ending, effectively replacing the Downer Ending in the movie. By this point Tony is assumed to have learned his bitter lessons from the events in the movie, though, and does show some good in him.
- Nightmare Fuel - For the most part, the game isn't scary at all. However, the chainsaw users are every bit as terrifying as Dr. Salvador. Especially if you're almost finished with a collection run and just have to save one more of your fronts before hitting the bank, kill most of the gangsters, but then hear that horrible revving and whirl around in a panic trying to kill the son of a bitch before he chops your head off and throws all your effort out the window.
- What happens when you swim too far out. It's not that bad if you know what's coming, but if you don't...
- Scrappy Mechanic: The cop system is despised by just about everyone who's played the game. If you aggravate the cops too much, you will be prompted to "Lose the cops!" and have to escape from them as the white Cop Exposure bar fills red. If it fills up all the way, as the game itself says, "You're fucked": The cops will just torment you to no end until you die, which you most likely will, very quickly. While it is possible to escape, it's very, very difficult. Especially later in the game when they start laying down tire shredders. Cops are on your tail? No problem, you can easily outrun them in your turbocharged Rattler. Run over the tire shredder (which is usually at every escape choke point) and it's worthless, forcing you to get out and jack someone's Jeep to make your escape.
- Tony's refusal to shoot civilians can cost some players a mission in a heated gun fight. Nothing is more annoying than shooting at some enemy gun men in the middle of the street and having Tony suddenly stop firing because an innocent woman is in the way. Naturally, enemies are not bound to this game mechanic and will gladly take advantage of it.
- Values Dissonance: Despite being optional, the side mission "Baggage Check" – whose name is a play on a person's genitals, and where Tony is tasked to check if the prostitutes at a motel are actually transgender by kneeing them in the groin – would not pass muster in this day and age due to it being both transphobic and insensitive towards sex workers, especially as the transgender sex workers are depicted in the most stereotypical way possible, the characters having effeminate male facial features and an equally effeminate male voice.
